INTRODUCTION TO HERPES
Even though herpes is not considered a life threatening disease, may patients suffering from severe case of genital herpes may wish they were dead. Herpes infection is a viral infection caused by herpes simplex virus. It is characterized by the appearance of group of vesicles, filled with clear fluid, on the skin or mucous membrane. There are two types of herpes simplex virus: herpes simplex virus 1 (HSV-1) and herpes simplex virus-2 (HSV-2). The area most commonly affected are lips, mouth, genital region, eyes, fingers, skin and in rare cases, it may affect the brain and internal organs.
A. Oral Herpes
Herpes infection of the oral cavity is commonly referred to as herpes, herpes simplex, cold sore, fever blister, herpes febrile or herpes labialis. It is estimated that 80-90% of herpes labialis is caused by HSV-1, and only 10-20% caused by HSV-2.
Oral herpes is generally transmitted via skin-skin or skin-mucous contact. It has been documented that the occurrence of oral herpes increases with crowded living conditions and lower socioeconomic status. For example, it was found that in the poor communities of Atlanta and Houston, approximately 33% of the children have herpes by age 5, and up to 80% have herpes by adolescence. In contract, the occurrence of herpes is much lower among individuals in middle class. Only 20% of children have herpes by age 5, there is usually no significant increase through adolescence. The incidence increases up to 40-60% by the second and third decades of life.
Primary infection generally occurs in young children less than 5 years old. Most individuals infected with primary herpes are asymptomatic, the primary herpes infection in young children is characterized by erythema, cluster of vesicles, ulceration, fever, malaise, headache, myalgia and possibly dehydration. In young adults, primary infection is characterized by pharyngitis, tonsilitis, painful clustered sore on the lip margin, fever, malaise and mononucleosis-like syndrome. The incubation period range from 2-12 days after the initial contact, the symptoms begin to manifest and the severity reaches it peak on days 3-4. Complete recovery may take 1 to 2 weeks.
After Primary herpes infection, the infected individuals may experience recurrent herpes two or more times per year. Usual precipitating factors for recurrence include skin/mucosal irritation, exposure to UV radiation, altered hormonal milieu (menstruation), fever, the common cold, altered immune states and stress. As with primary herpes infection, recurrent herpes may or may not be symptomatic. If symptomatic, recurrent herpes is generally characterized by a prodrome of tingling, itching, or burning sensation, which generally lasts for less than 6 hours. Within the next 24 to 48 hours, erythema and clusters of vesicles may develop, eventually becoming painful ulceration within 96 to 120 hours. Recovery is generally complete within 10 days, but way take up to 2 weeks. Systemic symptoms are usually absent in recurrent herpes.
B. Genital Herpes
Genital herpes is also referred to as herpes progenitalis, herpes genitalis and genital herpes simplex. Historically, 80-90% of genital herpes is caused by HSV-2, and only 10-20% caused byHSV-1. However, it is estimated that up to 30% of new cases of genital herpes is now caused by HSV-1.
Genital herpes is transmitted through the skin-skin sexual contact. Therefore, individuals who are sexually active with multiple sex partners, who have unprotected sex, are at high risk for contracting genital herpes. Genital herpes is most contagious when the host has identifiable lesions and /or ulcers. However, transmission may also occur during asymptomatic HSV shedding immediately before the appearance or after the healing of lesions.
It is estimated that 20% of adults in America have genital herpes. since genital herpes is not a reportable disease, this number could in fact be much higher. Among the population, genital herpes is tested positive in 70-80% of prostitutes, 46-60% in lower socio-economic individuals, 46% in homosexuals, and 25% in middle class individuals.
Primary genital herpes generally occurs in adolescence with young adults as these individuals become sexually active. After the initial contact, there is 2-7 days incubation period. The infected individuals will then experience fever, headache, malaise and myalgia. All peaking within first 3-4 days. In men, there may be clusters of vesicles on the penis or the penile shaft. They may also experience urethritis and dysuria. In homosexual men, lesions involving the rectal region are quite common. In women, there may be clusters of vesicles on vulva,perineum, buttocks or thighs. Presence of urinary discharge,dysuria or even urinary retention occurs frequently in women. In addition both men and women may experience inguinal lymphadenopathy, deep pelvic pain, pain radiating to the legs or into the lomyelitis leading to urinary retention, severe constipation and even neuralgia. Some cases of primary genital herpes become sever that hospitalization is required. complete recovery of primary genital herpes generally occurs within 3 weeks but may be longer if there is a complication. Precipitating factors for recurrence are generally associated with altered immune states or stress. Recurrent genital herpes is not as severe and is characterized by a prodrome of burning or itching sensation prior to eruptions of vesicles on the shaft of the penis or the male or simply a vulvar irritation in the female. Recurrent genital herpes last between 7 to 10 days. Neurologic or systemic complications are rare with recurrent genital herpes.
Herpes virus may also affects other parts of the body, such as eyes, fingers, skin, and in rare cases the brain and internal organs. Herpes infection of the eyes is called herpes keratitis. It is characterized by redness, tearing, pain and swelling of the eyelid and sudden onset of unilateral light sensitivity. Herpes eye infection is the most common cause of corneal blindness in the United States.
Herpes infection of the fingers is called herpes whitlow. It is characterized by a swollen, painful and erythematous lesion of the finger. It is transmitted through skin to skin contact when there is a skin break.
Herpes in the skin is called herpes gladiatorum, It occurs most frequently among wrestlers as there is an extensive skin to skin contact and the skin may not be 100%intact. Herpes gladiatorum may occur on any parts of the body.
Lastly, herpes infection of the brain is call herpes encephalitis. It is a rare complication of herpes, but it is the most common acute viral disease of the brain recognized in the United States. Early signs of herpes encephalitis include headache,nausea,vomiting, fever, chills and muscle ache. Patients may then experience olfactory hallucination, episode of irrational behavior, speech difficulties and seizure. If untreated, herpes encephalitis may lead to coma and death in several days in 70% of patients.
D. Treatment of Herpes in Western Medicine
The drug of choice for treating herpes is Zovirax (acyclovir), which may be taken internally or applied on the lesion topically. In oral herpes, internally administration of Zovirax was shown to reduce the time to "loss of crust" by approximately one day but did not alter the duration of pain or time to complete healing. Topical application of Zovirax did not alter the course of oral herpes and is not recommended.
In genital herpes, internal administration of Zovirax may shorten the time of healing and the duration of virus shedding by one day. Topical application of may also help in reducing the time of healing and duration of virus shedding as well. Zovirax is also available in intravenous form, but generally is reserve for severe case of primary herpes infection where hospitalization is needed.
Herpes in Oriental Medicine is called (Re Liao) "heat lesion" or (huo Liao) "burning-fire lesion" because the herpes lesions are hot, inflamed and extremely painful. Recurrent herpes may be precipitated by acute illness, fever, or lowered state of immunity.
A. Treatment of Oral Herpes with Chinese Herbal Formula
In Oriental medicine, oral herpes is due to exterior wind-heat attacking and residing in the lung and the stomach meridium. As heat travel upwards toward the mouth, lesions and eruptions appear in the oral cavity and around the lips. The lesions are red, hot ,inflamed and may be extremely painful.
Clinical manifestation of oral herpes include single or multiple clusters of small vesicles, filled with clear liquid found mainly around the lips and lath with tingling,itching,burning and/or needling sensation of the lesions, generally discomfort, dry stool, and yellow urine. The tongue is red with a thin yellow coating. The pulse is superficial slippery and rapid.
Treatment of oral herpes focuses on clearing the wind-heat, purging fire and detoxifying the heat-toxins. Treatment options include oral administration of herbs, topical application of herbs, herbal mouth rinse and acupuncture.

B. Treatment of Genital Herpes with Chinese Herbal Formula
In Oriental medicine, genital herpes is considered a syndrome of damp-heat residing in the Liver and the Gallbladder meridians, or excessive heat damaging the Yin. Damp-heat of the Liver and the Gallbladder channels manifest itself in the genital area as these channels pass through the region. Excessive heat may damage Yin,leading to genital herpes characterized by both excess heat and deficient Yin. Most primary genital herpes are considered as damp-heat of the Liver and the Gallbladder meridians, while recurrent herpes may be due to a combination of damp-heat of the Liver and the Gallbladder channels and Yin deficiency.
Genital Herpes due to damp-heat in the Liver and the Gallbladder meridian is characterized by small patches of erythema on the skin or mucous membranes proceeded by itching and soreness. Soon after painful vesicles appeared and form superficial circular ulcers with red areola. The painful ulcers crust and heal in approximately 10 days with scarring, accompanied by generalized malaize,fever, dysuria and irritability. The tongue should be red with a yellow tongue coating. The pulse should be rapid and possibly wiry or slippery. Treatment of genital herpes focuses on sedating fire, eliminating damp-heat from the Liver and Gallbladder channels, and nourish Yin. Treatment options include oral administration of herbs, topical application of herbs and acupuncture.
HERPES-G # 550 is the herbal formula of choice for genital herpes due to damp-heat in the Liver and Gallbladder meridians. This formula is extremely effective in reducing pain, inflammation and duration of genital herpes attacks, In fact most patients have reported up to 50% decrease in duration and severity of herpes attack using this formula.
